Transitional care takes place when a patient leaves his or her care setting and moves to another. It may also happen within and across healthcare settings and between healthcare providers. Competent transitional care matters because concerns may arise during or after moving patients.
Transitional care at Seba Home Care involves the assistance of a skilled caregiver in the transport of a patient from a facility to the home. Close monitoring of the patient’s condition is done for the next 72 hours to identify how much patients have adjusted to their new care setting.
Our caregivers give updates to the prescribing clinical team handling the patient’s treatment.
